The Wounds of Eventide update is the last major, free content update for Blasphemous. This update provides new bosses, enemies, areas, items, NPCs, and a new canon ending.

Unlocking the True Ending[]

The True Ending can be accessed through a normal playthrough or NG+. Unlocking this finale requires some careful steps throughout all the playthrough. If they are done incorrectly, the quest will become permanently blocked until a new game is started.

  1. Find Perpetva's grave on Echoes of Salt to obtain the Incomplete Scapular before encountering Esdras. Ten Piedad, Las Tres Angustias and Our Lady can be fought normally and in any order.
  2. Once you obtain the Incomplete Scapular, meet Esdras on the Bridge of the Three Calvaries. Perpetva will interrupt the start of the boss fight, and Esdras will let the Penitent One proceed without a fight. The player will receive the fight's normal rewards, but they will be disqualified from the Requiem Aeternam achievement. Esdras will tell for the Penitent One to meet him on the upper area of the Brotherhood, in the Mea Culpa Chapel.
  3. In there, Esdras will give the Key Grown from Twisted Wood, which can be used in the Library of the Negated Words. The game can be progressed normally, but you must complete the following steps before defeating Crisanta of the Wrapped Agony.
  4. Complete Diosdado's quest, which requires to find and interact with a ghost that's banging on a wall in the Library. He will give the The Young Masons Wheel as a reward. After this, interact with him back at the Prie Dieu and show him the Key, and he will open a secret door that leads to the imprisoned Fourth Visage.
  5. Speak with the Fourth Visage and proceed to defeat Sierpes and Isidora to obtain the Broken Left Eye of the Traitor and the Severed Right Eye of the Traitor in any order.
  6. Return the eyes to the Fourth Visage. The Penitent One will receive the Apodictic Heart of Mea Culpa. This Sword Heart is crucial to the ending, but cannot be removed once equipped.
  7. With the Apodictic Heart of Mea Culpa equipped, fight Crisanta of the Wrapped Agony as per usual. The item will change the boss fight by adding another phase, and completion will lead to a different outcome.
  8. Meet with Crisanta in the Mea Culpa Chapel. She will give the Holy Wound of Abnegation.
  9. Transform the Immaculate Bead into the Weight of True Guilt, then destroy all Statues of Confessors and complete all Confessor Dungeons. If you do not do this before defeating Escribar, you will get Ending B instead.
  10. Once all the Statues of Confessors are destroyed, and its dungeons beaten, fight Escribar as per usual. Once you climb to the throne on the mountain of ashes, a new area out of the map will be accessed. In there, the Final Boss of the DLC will be faced and the Ending C (True Ending), obtained.
